A SOOF lift blepharoplasty is an ingenious way to create a much younger appearance with a very small and subtle alteration to the face. To begin with, blepharoplasty is eyelid surgery. As we age, not only do crow’s feet form around the corners of our eyes, but a tiny deposit of fat under our eyelids called the suborbicular oculi fat (or SOOF) sinks to the tops of our cheeks as the fascial layer of tissue, which holds it in place, relaxes. This makes the eyes look wider than they are and creates puffy areas underneath them which are one of the lesser-known signs of aging.

 

Ordinary lid lifts and SOOF lifts

Traditionally, a surgeon will fix this by performing an open lower lid blepharoplasty, in which he or she makes an incision under the eyelashes of the lower lid, lifts the skin and muscle and takes the fat out completely. This does get rid of the wrinkles and puffiness, but the absence of the deposit of fat gives a hollow look to the lower eyelid and tear trough. Sometimes it is replaced by another deposit of fat taken from a different part of the body, but this carries a grotesque risk of its own — that the new lump of fat will prove too heavy for the lower lid and cause it to pull away from the eyeball.
