We can all tell when someone looks older, but apart from obvious differences like more wrinkles, we may not be able to describe precisely what the changes in appearance are. One of the things a facial plastic surgeon learns to do is understand the precise nature and cause of the changes and find ways to reverse them. For example, there is a tiny deposit of fat under our eyelids called the suborbicular oculi fat, or SOOF. It is held in place by a kind of connective tissue called the fascial layer. As we age, the fascial layer relaxes and the SOOF sinks to the tops of our cheeks. This creates a puffy appearance right under the eyes — a mark of increasing age that most people may not be consciously aware of.

 

A new approach

What surgeons usually do is perform an open lower lid blepharoplasty. This means making an incision under the eyelashes of the lower lid, lifting the skin and muscle and removing the fat. This gets rid of wrinkles, but it leaves the lower eyelid and tear through looking hollow. Sometimes the surgeon replaces this deposit of fat with a deposit of fat taken from another part of the body, but the effect does not always last, and the new deposit puts pressure on the support structure of the lower lid which can cause it to pull away from the eyeball. A better approach is the SOOF lift blepharoplasty. The surgeon makes an incision on the insider of the eyelid, returns the suborbicularis oculi fat to its original place and retightens the fascial layer to hold it there, reversing all the effects of aging on the eyelid in a single procedure. This subtle change in the structure of the face is much more effective at bringing back the original, youthful appearance of the eyes.
