Different forms of eyelid surgery

There are different forms of eyelid surgery, or blepharoplasty, for the upper and lower eyelids. This is because the upper and lower eyelids age in different ways. The upper eyelid has the problem of the brow sagging down onto it. The lower eyelid has the problem of a tiny deposit of fat under our eyelids called the suborbicularis oculi fat (or SOOF) that starts to slip out of place when the fascial layer of tissue loosens, causing it to sink to the tops of our cheeks. This makes the eyes look wide, and creates puffy areas underneath them. There are several ways of fixing this, some more effective than others.

The old way and the new way

The old way is to perform an open lower lid blepharoplasty. Here the surgeon makes an incision under the eyelashes of the lower lid, lifts the skin and muscle and takes the fat out completely.This does get rid of the wrinkles and puffiness, but the absence of the deposit of fat makes the lower eyelid and tear trough look kind of hollow. If the surgeon replaces the SOOFwith fat taken from a different part of the body, it carries the risk that the new lump of fat will be too heavy for the lower lid, creating a much worse situation.
